About the Community Health Survey

The NYC Community Health Survey (CHS) is an annual survey of 10,000 randomly selected adult New Yorkers. It is conducted by the New York City Department of Health and Mental Hygiene.

The survey helps the Health Department:

  • Track the health of New Yorkers
  • Guide health program decisions
  • Understand how different behaviors can affect a person’s health
  • Support health policy initiatives

The CHS gives the Health Department a better idea of the extent to which diseases, including COVID-19, and risk factors occur at the neighborhood, borough and city levels.

Respondents are randomly selected from a list of all available addresses in all five boroughs.

If you were selected to be part of the Community Health Survey and agree to participate, you will play a big role in representing your neighborhood, borough and New York City as a whole. Your participation is voluntary and all your answers will be confidential.

Abt Associates, a national research organization, is currently collecting the Community Health Survey data for the Health Department.

What is the New York City Community Health Survey? What will the survey ask me about?

The New York City Community Health Survey will ask you questions about your health and health practices, including the impact of the COVID-19 public health emergency. Your participation is voluntary. You can skip any question or stop the survey at any time. However, answering all of the questions will help the New York City Department of Health and Mental Hygiene better serve New Yorkers.

Can I complete the survey in other ways than on the Internet?

If you’d like to request a paper version of the survey or prefer to complete the survey over the phone, you can call 833-574-3096 and request that one be mailed to you or set up a time to complete it over the phone.

The survey is also available in Spanish, Chinese, Russian, Bengali, and Haitian Creole. Use the dropdown at the top of this page for information about the survey in these languages.

Do you ask personal questions?

Many of the questions are about personal health. Although some people may find that some answers may be difficult to share, we have no other way of knowing if a problem exists without asking about it. Your confidential answers help the NYC Health Department understand and address the health care needs of NYC.

Who in my household should complete the survey?

We ask for the adult (age 18 or older) in your household with the next birthday to take the survey. This helps make sure we are surveying a diverse group of New Yorkers. This way of choosing people randomly helps make sure the survey results reflect all New Yorkers.

How did you get my address?

Your address was randomly chosen from among all residential addresses in NYC.

How do I know this is an official NYC Health Department survey?

You can visit nyc.gov/health/nycsurveys for information about the survey. You can also contact our contracted vendor, Abt Associates, at 646-490-2091 or nycchs@abtassoc.com to make sure the survey is official. If you have any questions about this survey or survey results, you can call the NYC Health Department’s Project Manager at 347-396-2882. We are not selling anything or asking for money.

Is my information confidential? Will my privacy be protected?

Your information is handled in a secure and confidential manner. If you don’t want to take the survey, it will not affect any benefits you or your family get from any government agency. We do not ask about your citizenship or legal status.

Your survey answers are confidential. We will protect your privacy to the fullest extent of the law. Your answers cannot be used in federal, state or local legal proceedings without your consent. Your answers cannot be given to anyone not involved in the survey without your permission, unless allowed by law.

The survey has also been reviewed and approved by the NYC Health Department’s Institutional Review Board, which protects the rights of survey participants. To contact the Institutional Review Board, call 347-396-6118.

How long will the survey take?

The survey will take about 25 minutes to complete.

Why is the return address on the study envelope located in Massachusetts?

Abt Associates, the research organization contracted to collect the survey data for the NYC Health Department, processes all mail for the project out of its Cambridge, Massachusetts office.

How will I get paid for taking the survey?

After you take the survey, you will be sent a $25 electronic Visa gift card to the email address you give us within one or two business days. If you prefer, you can give us your mailing address and Abt Associates will send you a $25 Visa gift card in the mail within two to four weeks.
